A leading healthcare provider is seeking a dedicated Registered Nurse with strong OASIS experience for an immediate 13-week full-time home health contract. This position offers the chance to deliver high-quality care to patients in the comfort of their own homes while collaborating with a professional, supportive care team.

Schedule:

  • 13-week contract, with potential for extension
  • Full-time: Monday through Friday schedule
  • On-call duties may be required

Key Responsibilities:

  • Deliver skilled in-home nursing care, including wound care, medication management, patient education, and disease monitoring
  • Complete thorough patient assessments and detailed, timely OASIS documentation
  • Collaborate closely with physicians and interdisciplinary teams to create and execute individualized care plans
  • Monitor patient progress and promptly communicate any changes or concerns to the clinical team
  • Ensure patient instructions are clear, following up as needed to support ongoing health and recovery

Qualifications & Experience:

  • Active and unrestricted RN license
  • Minimum of 1 year recent experience as a home health RN
  • Demonstrated proficiency with OASIS documentation (required)
  • Familiarity with HomeCare HomeBase software (strongly preferred)
  • Proven ability to maintain an organized schedule and manage daily patient visits
  • Reliable transportation and a valid drivers license
