A leading healthcare provider is seeking a dedicated Registered Nurse with strong OASIS experience for an immediate 13-week full-time home health contract. This position offers the chance to deliver high-quality care to patients in the comfort of their own homes while collaborating with a professional, supportive care team.
Schedule:
- 13-week contract, with potential for extension
- Full-time: Monday through Friday schedule
- On-call duties may be required
Key Responsibilities:
- Deliver skilled in-home nursing care, including wound care, medication management, patient education, and disease monitoring
- Complete thorough patient assessments and detailed, timely OASIS documentation
- Collaborate closely with physicians and interdisciplinary teams to create and execute individualized care plans
- Monitor patient progress and promptly communicate any changes or concerns to the clinical team
- Ensure patient instructions are clear, following up as needed to support ongoing health and recovery
Qualifications & Experience:
- Active and unrestricted RN license
- Minimum of 1 year recent experience as a home health RN
- Demonstrated proficiency with OASIS documentation (required)
- Familiarity with HomeCare HomeBase software (strongly preferred)
- Proven ability to maintain an organized schedule and manage daily patient visits
- Reliable transportation and a valid drivers license
