We are seeking a dedicated Registered Nurse with extensive home health experience, specifically skilled in OASIS documentation, for a full-time, 13-week home health contract. This pivotal role delivers comprehensive care directly to patients homes, helping individuals thrive in their own environment.
Schedule:
- 13-week contract, with potential for extension
- Full-time, Monday through Friday
- On-call responsibilities are probable
Key Responsibilities:
- Deliver in-home skilled nursing care with a focus on patient safety and clinical excellence
- Perform comprehensive patient assessments and complete OASIS documentation accurately and within required timelines
- Collaborate closely with physicians and an interdisciplinary team to develop and implement individualized care plans
- Provide wound care, manage medications, educate patients and families, and monitor disease processes
- Maintain clear and timely communication regarding patient status changes with the clinical team
Qualifications:
- Active RN license
- Minimum one year of recent home health RN experience
- OASIS documentation experience required
- Proficiency with HomeCare HomeBase is strongly preferred
- Demonstrated ability to organize a structured daily schedule and efficiently manage multiple in-home visits
- Dependable transportation and a valid drivers license
