Schedule:
- 13-week contract with the possibility of extension
- Full-time, Monday through Friday
- On-call responsibilities may be required
- Structured daily visit expectations
Key Responsibilities:
- Deliver skilled nursing care and interventions in patient homes
- Perform comprehensive assessments with timely, accurate OASIS documentation
- Collaborate closely with physicians and interdisciplinary care teams to ensure individualized plans of care
- Provide wound care, medication management, patient/family education, and disease process monitoring
- Communicate updates or patient status changes promptly to appropriate clinical contacts
Qualifications & Experience:
- Active, unrestricted RN license
- At least one year of recent RN home health experience
- Strong proficiency with OASIS documentation (required)
- Experience with HomeCare HomeBase (strongly preferred)
- Proven success managing daily visit schedules
- Reliable transportation and valid drivers license
