Key Responsibilities:
- Deliver comprehensive nursing care in home settings, including initial and ongoing assessments.
- Monitor and record vital signs, manage medications, and provide wound care.
- Develop and tailor individualized care plans to support each patient's unique needs.
- Educate patients and families on disease management, treatment adherence, and preventive strategies.
- Collaborate closely with physicians, therapists, and interdisciplinary team members for optimal care.
- Maintain meticulous documentation of patient care in electronic medical records.
- Quickly recognize and report any significant changes in patient condition.
- Adhere to all applicable state and federal home health care regulations.
Schedule/Hours:
- Contract assignment with potential for extension based on performance.
Qualifications:
- Active RN license in good standing.
- At least 3 months of home health experience using OASIS assessment tools.
- Experience with HomeCare HomeBase (HCHB) is highly desirable.
- Demonstrated expertise in IV therapy, wound care, and medication management.
- Proficient in electronic medical records documentation.
- Exceptional communication and interpersonal abilities.
- Ability to work independently and efficiently manage caseload.
- Current drivers license and access to reliable transportation.
