Job Description
An opportunity is available for an experienced Registered Nurse (RN) specializing in home health to provide professional nursing care on a contract basis. This role requires a strong dedication to patient care and expertise in home health nursing, including experience with OASIS assessments.
Key Responsibilities:
- Deliver comprehensive nursing care to patients in their homes.
- Conduct initial and ongoing patient assessments, including vital signs, medication management, and wound care.
- Develop and implement individualized care plans tailored to patient needs.
- Educate patients and families on disease management, treatment plans, and preventive care.
- Collaborate with physicians, therapists, and other healthcare professionals to coordinate care.
- Accurately document all patient care activities in electronic medical records (EMR).
- Monitor and report any changes in patient condition to the supervising physician.
- Ensure adherence to all state and federal home health care regulations.
Qualifications / Desired Experience:
- Active Registered Nurse (RN) license.
- Minimum of 3 months home health experience utilizing OASIS.
- Experience with Homecare Homebase (HCHB) is a significant plus.
- Strong clinical skills, including IV therapy, wound care, and medication management.
- Proficient with electronic medical records systems.
- Excellent communication and interpersonal skills.
- Ability to work independently and manage time effectively.
- Valid drivers license and reliable transportation.
