An exciting opportunity is available for an experienced Registered Nurse with a home health background to provide high-quality care to patients in their homes. This contract role requires autonomy, compassion, and a firm commitment to excellent patient outcomes. OASIS experience is essential for this position, ensuring thorough assessments and top-tier care planning.
Schedule/Hours:
- Contract position; schedule details provided during interview process
Key Qualifications:
- Active RN license
- At least 3 months of recent home health experience utilizing OASIS
- Familiarity with HCHB (Highly valued)
- Demonstrated skills in IV therapy, wound care, and medication management
- Competency in electronic medical records (EMR) documentation
- Strong communication and organizational abilities
- Valid drivers license and dependable transportation
Primary Responsibilities:
- Deliver comprehensive nursing care in patients homes, adapting to changing clinical needs
- Execute initial assessments and perform ongoing monitoring of patient health, including wound and medication management
- Develop individualized care plans tailored to each patients needs and condition
- Provide clear education to patients and families on disease processes, treatment strategies, and preventive measures
- Collaborate with physicians, therapists, and the broader care team for seamless care coordination
- Maintain accurate, timely documentation in EMR systems
- Monitor patient status, report significant changes promptly, and comply with regulatory standards in home health care
