Your next nursing opportunity starts here. We are seeking an experienced Registered Nurse to provide essential home health services on a 13-week, full-time contract. Schedule:
- Full-time, Monday through Friday
- On-call responsibilities may be required
- 13-week contract duration, with strong possibility of extension
Role Responsibilities:
- Deliver skilled nursing care to patients in their homes following individualized care plans
- Conduct comprehensive patient assessments and complete accurate, timely OASIS documentation
- Perform wound care, medication administration, patient and caregiver education, and monitoring of chronic diseases
- Coordinate with physicians and healthcare teams to ensure seamless patient care
- Effectively communicate updates and patient status changes within the care team
- Maintain strict compliance with all home health protocols and quality standards
Desired Qualifications:
- Active RN license
- At least one year of recent home health nursing experience
- OASIS proficiency required
- Experience with HomeCare HomeBase is strongly preferred
- Valid drivers license and reliable transportation
- Strong organizational skills and ability to manage daily patient visit schedules
