An opening for a Registered Nurse (RN) with home health experience is available for a 13-week full-time contract in New Castle, IN. Schedule:
- Full-time, Monday through Friday

Responsibilities:
- Provide skilled nursing care and interventions in patient homes
- Conduct patient assessments and accurately complete OASIS documentation
- Collaborate closely with physicians and care teams to implement individualized care plans
- Perform wound care, medication management, patient education, and disease monitoring
- Communicate patient status updates to the clinical team promptly
Qualifications:
- Active RN license
- Minimum of one year of recent home health RN experience
- Required OASIS experience
- Proficiency with HomeCare HomeBase preferred
- Ability to manage a structured schedule with daily visits
- Reliable transportation and valid drivers license
