An opportunity is available for an experienced Registered Nurse (RN) specializing in home health care to provide essential nursing services on a contract basis in New Albany, IN. This role focuses on delivering high-quality patient care in home settings, emphasizing thorough assessments, patient education, and collaboration with multidisciplinary teams to foster positive health outcomes.
Responsibilities:
- Conduct comprehensive initial and ongoing patient assessments, including vital signs, medication management, and wound care.
- Develop and implement individualized care plans tailored to each patients needs.
- Educate patients and their families about disease management, treatment options, and preventive care.
- Collaborate closely with physicians, therapists, and other healthcare professionals to coordinate effective care.
- Accurately document all patient care activities in electronic medical records (EMR) in a timely manner.
- Monitor patient progress and report any significant changes to the supervising physician.
- Ensure adherence to all relevant state and federal regulations governing home health care.
Qualifications:
- Active RN license.
- Minimum of three months home health experience with demonstrated proficiency in OASIS.
- Knowledge of HomeCare HomeBase (HCHB) is highly advantageous.
- Strong clinical skills including IV therapy, wound care, and medication administration.
- Proficiency in using electronic medical records systems.
- Excellent communication and interpersonal abilities.
- Ability to work independently with effective time management.
- Valid drivers license and reliable transportation.
