Schedule/Hours:

Responsibilities:
- Provide skilled, in-home nursing care and interventions
- Perform thorough patient assessments and complete OASIS documentation accurately and punctually
- Collaborate with physicians and interdisciplinary care teams to implement personalized care plans
- Administer wound care, medication management, patient education, and chronic disease monitoring
- Communicate changes in patient condition to the clinical team promptly
Qualifications:
- Active Registered Nurse (RN) license
- Minimum one year recent experience in home health nursing
- Required experience with OASIS documentation
- Proficiency in HomeCare HomeBase is strongly preferred
- Ability to maintain a structured, full-time schedule with daily patient visits
- Reliable transportation and valid drivers license
