Deliver high-quality patient care in a home setting by joining a dedicated nursing team on a contract basis. This role is ideal for an experienced Registered Nurse (RN) seeking the opportunity to make a direct impact on patient lives while leveraging their expertise in home health.
Schedule/Hours:
- Contract position (duration discussed during interview)
Qualifications:
- Current, active RN license
- At least 3 months of home health nursing experience with required OASIS documentation skills
- Familiarity with HCHB (Homecare Homebase) preferred
- Strong background in IV therapy, wound care, and medication management
- Proficient with electronic medical records (EMR) systems
- Exceptional communication and interpersonal abilities
- Self-motivated with excellent time management
- Reliable transportation and valid drivers license
Key Responsibilities:
- Deliver comprehensive nursing care in patient homes, focusing on safety, quality, and comfort
- Conduct thorough assessments, including vitals, medication compliance, and wound evaluations
- Develop and execute patient-specific care plans, ensuring individualized attention
- Educate patients and family members on care procedures, chronic illness management, and preventive strategies
- Coordinate seamlessly with healthcare providers to maintain an integrated approach to patient care
- Accurately document and update patient records in EMR systems, ensuring timely communication of changes to supervising clinicians
- Adhere to regulatory standards for home health nursing
