This is a 13-week, full-time contract for a confident Registered Nurse with strong OASIS and documentation skills.
Schedule:
- Full-time, Monday through Friday
- On-call responsibilities are likely
- Defined daily visit quotas
Qualifications:
- Current, active Registered Nurse license
- At least 1 year of recent home health RN experience
- Proficiency with OASIS documentation
- Experience with HomeCare HomeBase (preferred)
- Reliable vehicle and valid drivers license
- Strong organizational and time-management abilities
Key Responsibilities:
- Deliver skilled nursing care and in-home interventions
- Conduct thorough patient assessments and timely, accurate OASIS documentation
- Collaborate with physicians and care team members to develop and follow individualized care plans
- Perform wound care, medication administration, patient education, and ongoing disease monitoring
- Relay significant clinical changes and updates clearly to the team
