Were seeking a dedicated Registered Nurse (RN) with proven OASIS experience for a 13-week full-time home health contract. This role is ideal for someone who excels in clinical skills and documentation, and values making a meaningful difference in patients lives through in-home care.
Schedule:
- Full-time, Monday through Friday
- Daily visit expectations
- Potential for on-call shifts
Key Responsibilities:
- Deliver skilled in-home nursing care, including wound care, medication management, patient education, and disease monitoring
- Conduct patient assessments and complete OASIS documentation accurately and promptly
- Collaborate with physicians and multidisciplinary care teams to develop and implement individualized patient care plans
- Monitor and communicate patient status, updating the team on changes in condition
- Maintain high standards of clinical excellence and compassionate care
Qualifications:
- Active RN license
- At least 1 year of recent experience as a home health RN
- Required OASIS assessment/documentation experience
- Proficiency with HomeCare HomeBase is strongly preferred
- Reliable transportation and valid drivers license
- Strong organizational skills and comfortable managing a structured, full-time visit schedule
