An exciting contract opportunity is available for an experienced and patient-focused Registered Nurse with a strong background in home health care. This role centers on delivering high-quality, comprehensive nursing services to patients in their homes, ensuring compassionate and effective care. OASIS experience is required for this position.
Schedule/Hours:
- Contract-based assignment
Qualifications:
- Active Registered Nurse (RN) license
- Minimum of 3 months of home health experience using OASIS
- HCHB (HomeCare HomeBase) proficiency is a significant advantage
- Strong clinical skills, including IV therapy, wound care, and medication management
- Proficient in electronic medical records (EMR)
- Excellent communication and interpersonal abilities
- Capable of independent work and effective time management
- Valid drivers license and dependable transportation
Key Responsibilities:
- Deliver comprehensive nursing care to patients within their homes
- Conduct initial and ongoing patient assessments, monitoring vital signs, medications, and wound healing
- Develop and execute individualized care plans tailored to specific patient needs
- Educate patients and their families on disease management, treatments, and prevention
- Work collaboratively with physicians, therapists, and other healthcare providers for coordinated care
- Accurately document all patient care actions and communications in the EMR system
- Observe, evaluate, and report any changes in patient status to supervising providers
- Ensure full compliance with state and federal home health regulations
