Seeking a dedicated Registered Nurse for a full-time, 13-week assignment in home health care. If you excel in providing skilled nursing in a home setting, demonstrate thorough OASIS documentation, and thrive in a structured environment, this is your chance to make a meaningful difference.
Schedule:
- 13-week contract with the option to extend
- Full-time, Monday through Friday
- On-call responsibilities may be required
Who You Are:
- Current, active RN license
- At least 1 year of recent home health RN experience
- Demonstrated OASIS documentation experience (required)
- Proficiency with HomeCare HomeBase strongly preferred
- Comfortable managing daily patient visits on a set schedule
- Reliable transportation and a valid drivers license
Key Responsibilities:
- Deliver in-home nursing care and skilled interventions
- Conduct patient assessments, completing OASIS documentation promptly and accurately
- Collaborate closely with physicians and interdisciplinary teams to design and execute individualized care plans
- Perform wound care, medication administration, patient education, and disease management
- Communicate relevant updates and any changes in patient condition to the broader clinical team
