An exciting contract opportunity is available for a Registered Nurse with a passion for home health. As a valued member of the care team, you will play a pivotal role in helping patients achieve their best health outcomes in the comfort of their homes. This role is best suited for an RN who thrives in independent work settings and enjoys building meaningful patient relationships.
Schedule/Hours:
- Contract basis (specific duration discussed during interview)
Key Responsibilities:
- Deliver comprehensive nursing care to home health patients
- Conduct initial and follow-up assessments, including monitoring vital signs and managing medications
- Perform wound care, IV therapy, and ensure accurate documentation within electronic medical records (EMR)
- Create personalized care plans tailored to each patients needs
- Educate patients and families on disease processes, treatments, and preventive strategies
- Collaborate closely with physicians, therapists, and healthcare professionals
- Monitor patient progress and promptly communicate any changes to the supervising physician
- Adhere to all state and federal regulations associated with home health care
Qualifications:
- Active Registered Nurse (RN) license
- At least 3 months of home health experience utilizing OASIS
- HCHB (Homecare Homebase) experience highly valued
- Skilled in IV therapy, wound care, and medication management
- Proficient in electronic charting and EMR systems
- Strong communication and interpersonal abilities
- Capable of working autonomously and managing time efficiently
- Valid drivers license and reliable vehicle required
