Schedule:
- 13-week contract with potential extension
- Monday through Friday, full-time hours
- On-call responsibilities possible
- Daily in-home patient visits
Qualifications:
- Current RN license in good standing
- At least 1 year of recent home health nursing experience
- Demonstrated OASIS proficiency
- Familiarity with HomeCare HomeBase (highly preferred)
- Valid drivers license and reliable transportation
- Excellent documentation habits and time management skills
Primary Responsibilities:
- Deliver skilled nursing care, interventions, and education to patients at home
- Complete thorough, timely OASIS assessments and documentation
- Develop and coordinate individualized patient care plans with physicians and interdisciplinary teams
- Provide wound care, medication teaching, and ongoing disease management
- Monitor and report patient progress or status changes promptly to the clinical team
