Deliver compassionate and expert care by supporting patients and families during their important end-of-life journey. A dedicated agency is seeking an experienced Registered Nurse with a background in home health and hospice to contribute expertise and empathy to their care team.
Schedule/Hours:
- Flexible scheduling depending on agency needs
Key Qualifications:
- Active and unrestricted RN license
- At least two years of recent hospice or palliative care experience required
- Proficient with HCHB (Homecare Homebase) software preferred
- Demonstrated strength in clinical assessment and communication
- Naturally compassionate, empathetic, and supportive
- Must have dependable transportation and a valid drivers license
Responsibilities:
- Deliver comprehensive nursing care to patients in their homes during hospice service
- Complete initial and ongoing patient assessments, develop individualized care plans, and document in compliance with agency protocols
- Coordinate with interdisciplinary hospice teams to ensure continuity of care for patients and their families
- Provide education and emotional support to patients, families, and caregivers
- Monitor patient symptoms and promptly communicate changes in condition or care needs
- Administer medications and treatments according to best practices and physician orders
