Deliver compassionate care to patients and families with a critical need for skilled support by stepping into a vital hospice nursing role. This contract opportunity focuses on providing expert care, comfort, and guidance to individuals in their homes throughout their end-of-life journey.
Qualifications and Skills:
- Active registered nurse (RN) license
- At least 2 years of direct hospice or palliative care nursing experience
- Proficiency with Homecare Homebase (HCHB) electronic documentation, preferred
- Reliable clinical assessment capabilities and effective communication skills
- Demonstrated empathy, patience, and compassionate bedside manner
- Dependable transportation and a valid drivers license
Key Responsibilities:
- Deliver comprehensive hospice care to patients in home settings, creating safe, comfortable environments
- Perform accurate assessments and develop individualized care plans in collaboration with physicians and multidisciplinary teams
- Educate patients and families about symptom management, medication, and disease progression
- Maintain clear, timely documentation through HCHB or other EMR systems
- Provide emotional support and guidance to patients, caregivers, and loved ones
- Coordinate with social workers, chaplains, and other hospice professionals to ensure holistic care
- Respond promptly to urgent needs and participate in on-call rotations, as required
