This full-time, 13-week contract position offers the chance to deliver quality nursing care in patients homes while maintaining structured daily visits. The role requires strong clinical skills, precise documentation, and the ability to manage a consistent schedule.

Key Responsibilities:
- Provide skilled in-home nursing care and interventions tailored to patient needs
- Conduct thorough patient assessments and complete OASIS documentation accurately and within deadlines
- Collaborate effectively with physicians and care teams to implement individualized care plans
- Perform wound care, medication management, patient education, and monitor disease progression
- Communicate patient condition updates promptly to the clinical team
Qualifications and Desired Experience:
- Active Registered Nurse (RN) license
- Minimum of one year recent experience in home health nursing
- Proficient in OASIS documentation (required)
- Experience with HomeCare HomeBase software preferred
- Ability to manage a full-time, structured schedule with daily home visits
- Reliable transportation and valid drivers license essential
