We're seeking a dedicated Registered Nurse (RN) with strong home health experienceincluding proficiency with OASIS documentationfor a 13-week, full-time contract. This role is ideal for nurses confident in both clinical care and detailed patient documentation, ready to excel in a structured home health setting.
Schedule:
- Full-time, Monday through Friday
- Potential on-call responsibilities
Key Responsibilities:
- Deliver skilled in-home nursing care and interventions
- Complete patient assessments and OASIS documentation accurately and promptly
- Collaborate with physicians and interdisciplinary teams to develop personalized care plans
- Provide wound care, medication management, patient education, and disease monitoring
- Communicate any changes in patient conditions to the clinical team
Qualifications:
- Active RN license
- At least 1 year of recent experience as a home health RN
- Proficiency in OASIS assessments
- Experience with HomeCare HomeBase is strongly preferred
- Reliable transportation and a valid drivers license
- Ability to manage and coordinate a full-time visit schedule
