Job Description

Your nursing expertise meets purpose in this full-time, 13-week contract role specializing in home health care. As a Registered Nurse (RN) with a background in OASIS and home health, you will play a vital role in supporting patients and families during crucial times, helping drive improved clinical outcomes while maintaining quality standards. This assignment offers you the chance to deliver one-on-one, in-home patient care, while collaborating closely with a professional, supportive clinical team.

Schedule:

  • Full-time, Monday through Friday
  • On-call may be required

Key Responsibilities:

  • Deliver skilled nursing care and interventions in patients homes
  • Complete accurate, timely patient assessments and OASIS documentation
  • Develop and implement individualized care plans in collaboration with physicians and interdisciplinary care teams
  • Perform wound care, medication administration, patient/family education, and monitor ongoing disease management
  • Provide thorough updates and communicate changes in patient condition to clinical teams
  • Maintain documentation integrity and adhere to visit guidelines

Who Were Seeking:

  • Active RN license
  • At least one year of recent RN experience in a home health setting
  • Strong proficiency with OASIS documentation
  • Familiarity with HomeCare HomeBase (preferred)
  • Reliable vehicle and valid drivers license
  • Able to independently manage a structured daily visit schedule
