Your next nursing opportunity begins now with an engaging contract role for an experienced Registered Nurse in home health. This is a full-time, 13-week assignment focused on delivering skilled care to patients in their homes while utilizing your expertise and OASIS proficiency to ensure exceptional outcomes.
Schedule:
- Full-time, Monday through Friday
- On-call likely required
- Daily structured patient visits
Key Responsibilities:
- Deliver comprehensive in-home nursing care, including wound care, medication management, and disease education
- Perform patient assessments and complete OASIS documentation precisely and promptly
- Collaborate closely with interdisciplinary care teams to develop and implement individualized treatment plans
- Communicate patient progress and any changes to the clinical team to ensure continuity of care
- Travel between patient homes, maintaining punctuality and professionalism
Qualifications:
- Active RN license
- At least 1 year of recent home health RN experience
- Mandatory OASIS experience
- Strong proficiency with HomeCare HomeBase (preferred)
- Confident managing a full-time patient schedule independently
- Reliable transportation and valid drivers license
