Key Responsibilities:
- Deliver thorough nursing care to patients within their homes.
- Conduct initial and ongoing assessments, including vital signs monitoring, medication management, and wound care.
- Develop and implement tailored care plans that address individual patient needs.
- Educate patients and their families about disease management, treatment protocols, and preventive health strategies.
- Collaborate closely with physicians, therapists, and healthcare team members to optimize patient care.
- Maintain accurate and timely documentation of all nursing interventions in electronic medical records (EMR).
- Monitor patient conditions vigilantly and communicate any changes to the supervising physician.
- Comply with applicable state and federal regulations governing home health services.
Qualifications:
- Current and active RN license.
- Minimum of three months home health experience with proficiency in OASIS.
- Familiarity with Homecare Homebase (HCHB) software is highly advantageous.
- Strong clinical skills including IV therapy, wound care, and medication administration.
- Proficient in electronic medical records (EMR) systems.
- Excellent communication and interpersonal capabilities.
- Self-directed with strong time management skills.
- Valid drivers license and reliable transportation.
