A fulfilling, impactful role awaits a dedicated Registered Nurse with strong home health experience and OASIS proficiency. This full-time, 13-week contract offers you the chance to contribute meaningful care to patients in their homes and collaborate with a skilled team of healthcare professionals.
Schedule:
- Full-time, Monday through Friday
- On-call responsibilities may be required
Key Responsibilities:
- Deliver in-home skilled nursing care and interventions tailored to each patients needs
- Perform comprehensive assessments and submit OASIS documentation accurately and promptly
- Maintain regular communication with physicians and interdisciplinary care teams to ensure coordinated care
- Provide wound care, patient and family education, medication management, and monitor disease symptoms
- Accurately report changes in patient condition to the clinical team and update care plans as needed
Qualifications:
- Current RN license
- At least one year of recent home health nursing experience required
- Proven OASIS documentation skills
- Proficiency with HomeCare HomeBase (highly preferred)
- Strong organizational skills for managing daily home visits
- Reliable transportation and valid drivers license
