An excellent opportunity is available for a skilled and compassionate Registered Nurse with home health experience. This contract role is perfect for RNs who excel at delivering high-quality care to patients in a home setting and thrive in an autonomous environment. OASIS competency is a mandatory requirement for this position.
Schedule/Hours:
- Flexible contract schedule (details will be provided during the interview process)
Qualifications:
- Current and active RN license
- At least 3 months of home health nursing experience utilizing OASIS documentation
- Experience with HCHB (Homecare Homebase) is highly valued
- Strong clinical capabilities in IV therapy, wound care, and medication management
- Proficiency using electronic medical record (EMR) systems
- Excellent interpersonal and communication abilities
- Self-motivated with strong time management skills
- Valid drivers license and reliable vehicle
Key Responsibilities:
- Deliver comprehensive nursing care directly in patients homes, ensuring a high level of independence and responsibility
- Conduct both initial and ongoing assessments, including vital signs monitoring and wound management
- Create, implement, and update individualized care plans based on patient condition and needs
- Educate patients and families on health management, treatment protocols, and preventive strategies
- Collaborate closely with physicians and allied health professionals to provide integrated, cohesive care
- Accurately document all patient interactions and care procedures in EMR systems
- Promptly report changes in patient status to the supervising physician
- Adhere to all applicable home health regulations and compliance standards
