A Registered Nurse (RN) with a passion for home health care is needed for a contract role in Lexington, KY. This position focuses on delivering high-quality, patient-centered care in the comfort of patients homes, emphasizing comprehensive assessments and individualized care planning. Candidates must have OASIS experience to be considered.
Key Responsibilities:
- Provide thorough nursing care in patients' homes, including vital signs, medication management, and wound care
- Conduct initial and ongoing patient assessments and develop personalized care plans
- Educate patients and families on disease management, treatment plans, and preventive care
- Collaborate with physicians, therapists, and other healthcare professionals to ensure integrated care
- Accurately document all patient care activities promptly in electronic medical records (EMR)
- Monitor patient conditions and report significant changes to supervising physicians
- Maintain compliance with all state and federal home health regulations
Qualifications:
- Active RN license
- Minimum of 3 months of home health experience with OASIS required
- Experience with HCHB (Homecare Homebase) is a strong plus
- Strong clinical skills, including IV therapy, wound care, and medication management
- Proficient in EMR systems
- Excellent communication and interpersonal abilities
- Self-motivated with strong time management skills
- Valid drivers license and reliable transportation
