A dedicated Registered Nurse with OASIS experience is needed for a 13-week home health assignment. Step into a pivotal role where precision, professionalism, and compassionate care are at the heart of each day. This full-time contract offers the chance to utilize your clinical expertise and accuracy in documentation while delivering exceptional in-home nursing services.
Schedule:
- 13-week assignment, with the potential to extend
- Full-time hours, Monday through Friday
- Anticipated on-call responsibilities
- Structured daily patient visit expectations
Key Responsibilities:
- Deliver in-home nursing care and skilled interventions to home health patients
- Complete comprehensive patient assessments, ensuring accurate and timely OASIS documentation
- Provide wound care, medication management, patient education, and chronic disease monitoring
- Collaborate closely with physicians and multidisciplinary care teams to establish and implement individualized care plans
- Communicate effectively with the clinical team regarding updates and changes in patient condition
Qualifications:
- Active and unencumbered RN license
- At least 1 year of recent home health nursing experience
- Demonstrated OASIS proficiency is required
- HomeCare HomeBase experience strongly preferred
- Proven ability to manage structured visit schedules independently
- Reliable personal transportation and a valid driver's license
