An excellent opportunity is available for an experienced Registered Nurse with a background in home health. This contract position is ideal for professionals who are passionate about delivering personalized patient care and thrive in an autonomous environment. OASIS experience is required.
Schedule/Hours:
- Contract position with the potential for extension based on performance
Key Responsibilities:
- Deliver comprehensive, compassionate nursing care to patients in their homes
- Conduct initial and ongoing assessments, including vital signs, medication management, and wound care
- Develop and implement individualized care plans tailored to patient needs
- Provide patient and family education on disease management and preventive care
- Collaborate with physicians, therapists, and other healthcare professionals to coordinate care
- Maintain precise and timely documentation in electronic medical records (EMR)
- Monitor and communicate changes in patient condition to supervising physicians
- Ensure compliance with all relevant state and federal regulations
Qualifications:
- Active RN license
- At least 3 months of recent home health experience utilizing OASIS
- HCHB experience highly desirable
- Strong clinical skills in IV therapy, wound care, and medication management
- Proficiency with EMR systems
- Excellent communication and interpersonal skills
- Capable of working independently and managing a flexible workload
- Valid drivers license and access to reliable transportation
