A compassionate, detail-oriented Registered Nurse with home health expertise is needed for a 13-week full-time contract. This rewarding assignment is ideal for an RN with proven OASIS documentation skills, a structured approach to daily scheduling, and a commitment to high-quality patient care.
Schedule & Location:
- Full-time, Monday through Friday
- On-call responsibilities may be required
- Structured visit volume each day
Key Responsibilities:
- Deliver skilled in-home nursing care and interventions
- Perform comprehensive patient assessments and complete OASIS documentation accurately and on time
- Collaborate closely with physicians and interdisciplinary care teams to implement individualized care plans
- Provide wound care, medication administration, disease management, and essential patient/caregiver education
- Monitor and communicate changes in patient condition to the clinical team
Qualifications:
- Active RN license in good standing
- At least 1 year of recent RN home health experience
- Proficient with OASIS (required)
- Experience with HomeCare HomeBase (strongly preferred)
- Dependable, organized, and comfortable managing a daily visit schedule
- Reliable transportation and valid drivers license
