A fulfilling position is available for a Registered Nurse with comprehensive home health and OASIS experience. This 13-week, full-time contract offers the chance to deliver meaningful care to patients in their own homes while working closely with a dedicated clinical team.
Schedule:
- Full-time, Monday through Friday
- On-call rotation likely
What to Expect:
- 13-week contract with extension opportunities
- Structured patient visit schedule
Key Responsibilities:
- Deliver in-home nursing care, including skilled interventions and patient education
- Conduct thorough patient assessments and complete OASIS documentation promptly and accurately
- Collaborate with physicians and interdisciplinary teams to customize care plans
- Perform wound care, manage medications, and monitor patient conditions
- Ensure timely communication of patient status and updates to the clinical team
Who You Are:
- Active, unrestricted RN license
- Minimum one year of recent experience as a home health RN
- Demonstrated proficiency with OASIS documentation
- Strongly prefer experience with HomeCare HomeBase
- Highly organized and capable of managing daily visit expectations
- Dependable transportation and a current drivers license
