A valued opportunity is available for an experienced Registered Nurse with a strong foundation in home health. This temporary role centers on delivering patient-focused care within the comfort of clients homes, ensuring safety, comfort, and individualized treatment based on nursing best practices.
Schedule/Hours:
- Contract basis (assignment duration details provided during the interview process)
Qualifications:
- Active RN license
- At least 3 months of home health experience (OASIS experience required)
- HCHB (Homecare Homebase) experience highly desirable
- Demonstrated skills in IV therapy, wound care, and medication management
- Proficiency with electronic medical record (EMR) documentation
- Strong communication and interpersonal abilities
- Valid driver's license and reliable transportation
Key Responsibilities:
- Deliver comprehensive nursing care directly in patients homes, including routine assessments and care plan development
- Complete in-depth initial and ongoing assessments of each patients condition, addressing vital signs, medication regimes, and wound care
- Educate patients and family members on disease processes, treatment goals, and preventative health
- Collaborate closely with physicians, therapists, and interdisciplinary team members to coordinate and revise care plans
- Maintain timely and accurate documentation in EMR systems, ensuring compliance with regulatory standards
- Report any notable changes in patient condition promptly to medical supervisors
- Ensure adherence to home health care standards at both the state and federal levels
