A rewarding nursing opportunity is available for a Registered Nurse (RN) specializing in home health care with OASIS experience. This 13-week full-time contract position offers a structured schedule, allowing you to deliver skilled nursing care in patients homes while making a meaningful impact.

Responsibilities:
- Provide skilled nursing care and clinical interventions in patients homes
- Conduct comprehensive patient assessments and ensure timely, accurate completion of OASIS documentation
- Collaborate closely with physicians and interdisciplinary care teams to develop and implement individualized care plans
- Perform wound care, medication management, patient education, and monitor disease progression
- Communicate patient status updates and any changes to the healthcare team effectively
Qualifications:
- Active RN license
- Minimum of 1 year recent home health nursing experience
- Proven proficiency with OASIS documentation is required
- Experience with HomeCare HomeBase is highly preferred
- Ability to manage a structured full-time schedule with daily patient visits
- Reliable transportation and a valid drivers license are essential
Schedule:
- Full-time, Monday through Friday with on-call responsibilities possible
