An opportunity is available for an experienced Registered Nurse (RN) with a focus in home health care on a contract basis. This role requires a compassionate and skilled nurse committed to delivering exceptional patient care in a home setting, using proven assessment and care planning expertise.
Key Responsibilities:
- Deliver comprehensive nursing care to patients within their homes
- Conduct initial and ongoing patient assessments including vital signs, medication management, and wound care
- Develop and implement personalized care plans tailored to individual patient needs
- Educate patients and families regarding disease management, treatment protocols, and preventive care
- Collaborate with physicians, therapists, and healthcare professionals to maintain a cohesive care approach
- Accurately document patient care activities in electronic medical records (EMR)
- Monitor patient condition changes, reporting them promptly to supervising physicians
- Ensure adherence to state and federal regulations governing home health care
Qualifications:
- Active Registered Nurse license
- Minimum of 3 months experience in home health nursing with strong proficiency in OASIS assessments
- Experience with Homecare Homebase (HCHB) software is highly desirable
- Skilled in IV therapy, wound care, and medication management
- Proficient with electronic medical records systems
- Excellent communication and interpersonal skills
- Self-motivated with strong time management abilities
- Valid drivers license and reliable transportation
