Job Description

An opportunity is available for an experienced Registered Nurse (RN) with a focus in home health care on a contract basis. This role requires a compassionate and skilled nurse committed to delivering exceptional patient care in a home setting, using proven assessment and care planning expertise.

Key Responsibilities:

  • Deliver comprehensive nursing care to patients within their homes
  • Conduct initial and ongoing patient assessments including vital signs, medication management, and wound care
  • Develop and implement personalized care plans tailored to individual patient needs
  • Educate patients and families regarding disease management, treatment protocols, and preventive care
  • Collaborate with physicians, therapists, and healthcare professionals to maintain a cohesive care approach
  • Accurately document patient care activities in electronic medical records (EMR)
  • Monitor patient condition changes, reporting them promptly to supervising physicians
  • Ensure adherence to state and federal regulations governing home health care

Qualifications:

  • Active Registered Nurse license
  • Minimum of 3 months experience in home health nursing with strong proficiency in OASIS assessments
  • Experience with Homecare Homebase (HCHB) software is highly desirable
  • Skilled in IV therapy, wound care, and medication management
  • Proficient with electronic medical records systems
  • Excellent communication and interpersonal skills
  • Self-motivated with strong time management abilities
  • Valid drivers license and reliable transportation
