Key Responsibilities:
- Deliver comprehensive nursing care tailored to individual patient needs, including assessments such as vital signs, medication administration, and wound care.
- Develop and implement personalized care plans, educating patients and families about disease management, treatment options, and prevention strategies.
- Collaborate effectively with physicians, therapists, and other healthcare team members to coordinate care.
- Accurately document all patient care activities in electronic medical records (EMR) systems.
- Monitor patient progress, communicate condition changes promptly, and ensure compliance with all regulatory requirements in home health care.
Qualifications:
- Active Registered Nurse license.
- Minimum of 3 months home health experience with proficiency in OASIS.
- Hands-on skills in IV therapy, wound care, and medication management.
- Experience with electronic medical records and preferably Homecare Homebase (HCHB).
- Strong communication abilities and interpersonal skills.
- Self-motivated with excellent time management capabilities.
- Valid drivers license and dependable transportation.
