A Registered Nurse (RN) with home health experience is sought for a contract role in Ingalls, MI. This position centers around delivering compassionate, patient-focused care in a home setting, requiring proficiency in OASIS documentation and a strong clinical skill set.
Responsibilities:
- Conduct thorough initial and ongoing patient assessments, including vital signs, medication management, and wound care.
- Develop and execute individualized care plans tailored to patient needs.
- Educate patients and their families on disease management, treatment protocols, and preventative health measures.
- Collaborate effectively with physicians, therapists, and other healthcare professionals to ensure integrated care plans.
- Accurately document all patient care activities using electronic medical records (EMR).
- Monitor patient conditions closely and communicate any changes to supervising physicians.
- Adhere strictly to all applicable state and federal home health care regulations.
Qualifications:
- Active Registered Nurse (RN) license.
- Minimum of three months home health experience utilizing OASIS.
- Experience with Home Care Home Base (HCHB) software is a strong advantage.
- Solid clinical skills, including intravenous (IV) therapy, wound care, and medication management.
- Proficient in electronic medical records (EMR) systems.
- Strong communication and interpersonal abilities.
- Self-motivated with excellent time management skills.
- Valid drivers license and dependable transportation.
