Job Description

A Registered Nurse (RN) with home health experience is sought for a contract role in Ingalls, MI. This position centers around delivering compassionate, patient-focused care in a home setting, requiring proficiency in OASIS documentation and a strong clinical skill set.

Responsibilities:

  • Conduct thorough initial and ongoing patient assessments, including vital signs, medication management, and wound care.
  • Develop and execute individualized care plans tailored to patient needs.
  • Educate patients and their families on disease management, treatment protocols, and preventative health measures.
  • Collaborate effectively with physicians, therapists, and other healthcare professionals to ensure integrated care plans.
  • Accurately document all patient care activities using electronic medical records (EMR).
  • Monitor patient conditions closely and communicate any changes to supervising physicians.
  • Adhere strictly to all applicable state and federal home health care regulations.

Qualifications:

  • Active Registered Nurse (RN) license.
  • Minimum of three months home health experience utilizing OASIS.
  • Experience with Home Care Home Base (HCHB) software is a strong advantage.
  • Solid clinical skills, including intravenous (IV) therapy, wound care, and medication management.
  • Proficient in electronic medical records (EMR) systems.
  • Strong communication and interpersonal abilities.
  • Self-motivated with excellent time management skills.
  • Valid drivers license and dependable transportation.
