A dedicated Registered Nurse with home health expertise is needed for a 13-week full-time contract assignment supporting patient care in the home setting. This is an opportunity to leverage your OASIS experience, clinical proficiency, and attention to documentation detailmaking an immediate and meaningful difference in patients lives.
Schedule:
- Full-time, Monday through Friday
- On-call responsibilities may be included
- Structured daily patient visit expectations
What Youll Do:
- Deliver skilled in-home nursing care and interventions
- Conduct thorough patient assessments and complete OASIS documentation accurately and promptly
- Collaborate with interdisciplinary care teams to formulate and execute individualized care plans
- Provide wound care, manage medications, deliver disease education, and monitor patient progress
- Maintain clear communication regarding changes in patient condition with the clinical team
Qualifications:
- Active RN license
- At least one year of recent home health RN experience
- OASIS documentation experience required
- Proficiency with HomeCare HomeBase strongly preferred
- Reliable transportation and valid drivers license
- Committed to managing a full-time, structured caseload
