Responsibilities:
- Provide skilled nursing care and interventions in patients homes.
- Conduct thorough patient assessments and complete OASIS documentation accurately and timely.
- Collaborate with physicians and healthcare teams to develop and implement individualized care plans.
- Perform wound care, medication management, patient education, and disease monitoring.
- Communicate patient status updates and any changes in condition to the clinical team.
Qualifications:
- Active RN license.
- Minimum of one year recent experience in home health nursing.
- Proficiency with OASIS documentation is required.
- Familiarity with HomeCare HomeBase software is strongly preferred.
- Ability to manage a structured full-time schedule with daily patient visits.
- Reliable transportation and a valid drivers license are essential.
Schedule:
- Full-time, Monday through Friday.
- On-call duties are probable.
