An opportunity is available for a skilled Registered Nurse (RN) specializing in home health nursing on a contract basis. This role focuses on delivering high-quality, patient-centered care in the home setting and requires experience with OASIS documentation.
Key Responsibilities:
- Provide comprehensive nursing care to patients in their homes, including vital signs monitoring, medication management, and wound care.
- Conduct initial and ongoing patient assessments.
- Develop and implement individualized care plans tailored to patient needs.
- Educate patients and families on disease management, treatment plans, and preventive care.
- Collaborate closely with physicians, therapists, and healthcare professionals to ensure coordinated care.
- Document all patient care activities accurately and promptly in electronic medical records (EMR).
- Monitor and report changes in patient condition to supervising physicians.
- Ensure compliance with state and federal home health care regulations.
Qualifications:
- Active RN license.
- Minimum of three months of home health experience utilizing OASIS.
- Experience with HCHB (Home Care Home Base) EMR is a plus.
- Strong clinical skills, including IV therapy, wound care, and medication management.
- Proficient with electronic medical records systems.
- Excellent communication and interpersonal skills.
- Ability to work independently and manage time effectively.
- Valid drivers license and reliable transportation.
