• Ability to use EMR systems effectively
• Excellent interpersonal and communication skills
• Organized, self-directed, and efficient in time management
• Valid drivers license and reliable, insured transportation

Primary Responsibilities:

• Deliver comprehensive nursing interventions in home settings
• Conduct thorough patient assessmentsvital signs, medication oversight, wound evaluation
• Create and update personalized care plans in collaboration with the healthcare team
• Educate patients and families on disease processes, care protocols, and preventive practices
• Maintain timely and accurate documentation in EMR
• Observe and promptly report any change in patient conditions
