A focused opportunity is available for a Registered Nurse (RN) with home health expertise and OASIS experience to provide high-quality care in a full-time, 13-week contract role. This position is designed for a nurse confident in clinical assessments, skilled nursing interventions, and precise documentation who thrives within a structured schedule.

The role is located in Huntingdon, PA, a community known for its friendly atmosphere and beautiful natural surroundings, including proximity to the Juniata River and scenic outdoor recreation. This area offers a balance of small-town charm and accessibility to larger cities.
Key Responsibilities:
- Deliver skilled nursing care and interventions in patients homes
- Conduct thorough patient assessments and complete accurate OASIS documentation timely
- Collaborate closely with physicians and interdisciplinary care teams to develop personalized care plans
- Manage wound care, medication administration, patient teaching, and chronic disease monitoring
- Communicate patient progress and any changes in condition to the clinical team
Qualifications:
- Active RN license in good standing
- Minimum of 1 year recent experience in home health nursing
- Proven experience with OASIS documentation is required
- Familiarity with HomeCare HomeBase software is strongly preferred
- Ability to maintain a full-time schedule with daily patient visits
- Reliable transportation and valid drivers license
Schedule:
- Full-time, Monday through Friday
- On-call availability probable
