Your next nursing opportunity awaits as a Registered Nurse specializing in Home Health for a 13-week full-time contract. In this critical position, youll leverage your clinical acumen, mastery of OASIS documentation, and passion for patient-centered care to serve individuals where theyre most comfortabletheir homes.
Schedule:
- 13-week contract with the potential for extension
- Full-time hours, Monday through Friday
- Daily visit expectations
- On-call duties may be required
Qualifications:
- Active RN license
- At least 1 year of recent experience in home health nursing
- Demonstrated OASIS documentation experience
- Proficiency with HomeCare HomeBase (strongly preferred)
- Reliable transportation and a valid driver's license
- Ability to manage a structured, full-time schedule and meet daily patient visit requirements
Key Responsibilities:
- Deliver comprehensive in-home nursing care and skilled interventions
- Conduct patient assessments with timely, accurate OASIS documentation
- Collaborate closely with physicians and interdisciplinary care teams to devise individualized care plans
- Provide wound care, medication management, patient and family education, and ongoing condition monitoring
- Communicate patient updates and changes promptly to the clinical team
