Your next nursing opportunity starts here with a full-time contract role for a dedicated Registered Nurse (RN) specializing in home health care. This 13-week assignment is ideal for an RN skilled in OASIS documentation and passionate about delivering high-quality care in patients homes.
Schedule:
- Full-time, Monday through Friday
- On-call responsibilities may be required
- Daily visit structure for consistent workflow
Key Responsibilities:
- Deliver professional in-home nursing care, including skilled interventions and patient education
- Perform comprehensive assessments and complete OASIS documentation accurately and punctually
- Collaborate with physicians and multidisciplinary care teams to create and update individualized care plans
- Manage wound care, medication administration, and chronic disease monitoring
- Communicate promptly regarding any patient status changes to ensure continuity of care
Required Qualifications:
- Active and unencumbered RN license
- Minimum of 1 year recent experience as a home health RN
- Demonstrated proficiency with OASIS documentation
- Knowledge of HomeCare HomeBase (strongly preferred)
- Ability to work a structured, full-time schedule with reliable daily patient visits
- Valid drivers license and reliable transportation
