An opportunity is available for a skilled Registered Nurse (RN) with expertise in home health to provide compassionate and comprehensive nursing care in patients residences. This contract position calls for dedication to quality patient outcomes and familiarity with OASIS assessments.
Key Responsibilities:
- Deliver thorough nursing care tailored to the unique needs of home health patients.
- Conduct initial and ongoing patient assessments, including vital signs, wound care management, and medication administration.
- Develop and implement individualized care plans to promote patient health and recovery.
- Educate patients and their families on disease processes, treatment adherence, and preventive strategies.
- Collaborate closely with physicians, therapists, and other healthcare team members to ensure seamless patient care.
- Maintain timely, accurate documentation of all care activities in electronic medical records (EMR).
- Monitor patient status and promptly report clinical changes to supervising physicians.
- Adhere to all applicable state and federal regulations governing home health nursing.
Qualifications:
- Active Registered Nurse (RN) license.
- At least 3 months of home health nursing experience with proven OASIS proficiency.
- Experience with Homecare Homebase (HCHB) software is highly desirable.
- Strong clinical competencies, including IV therapy, wound care expertise, and medication management.
- Proficient with EMR systems and technology tools.
- Excellent communication skills and ability to work independently.
- Valid drivers license with reliable transportation for travel to patient homes.
