Responsibilities:
- Deliver skilled nursing care and interventions during in-home visits
- Conduct thorough patient assessments and ensure accurate, timely OASIS documentation
- Collaborate closely with physicians and multidisciplinary teams to implement tailored care plans
- Perform wound care, medication administration, patient education, and monitor chronic conditions
- Maintain open communication regarding patient status with the clinical team
Qualifications:
- Active Registered Nurse (RN) license
- Minimum one year of recent home health RN experience
- Required proficiency with OASIS documentation
- Experience with HomeCare HomeBase is highly preferred
- Ability to follow a structured schedule with daily visits, Monday through Friday
- Reliable transportation and valid drivers license
