An opportunity is available for an experienced Registered Nurse (RN) with a strong background in home health to provide compassionate, patient-centered care in Harvest, AL. This contract role is ideal for RNs skilled in comprehensive nursing care within patients homes, utilizing OASIS for assessments and documentation.
Key Responsibilities:
- Deliver thorough nursing care including initial and ongoing patient assessments.
- Monitor vital signs, manage medications, and perform wound care.
- Develop and implement personalized care plans tailored to patient needs.
- Educate patients and families on disease management, treatment, and prevention.
- Collaborate with physicians, therapists, and other healthcare team members to coordinate care.
- Accurately document all care activities in electronic medical records (EMR).
- Observe and report changes in patient condition to supervising physicians.
- Ensure adherence to state and federal home health care regulations.
Qualifications:
- Active Registered Nurse (RN) license.
- Minimum three months home health experience with OASIS.
- Experience with HCHB electronic health records is advantageous.
- Strong clinical proficiency in IV therapy, wound care, and medication management.
- Familiarity with EMR systems.
- Effective communication and interpersonal skills.
