A dedicated Registered Nurse is needed for a full-time home health contract role. This position offers the chance to make a significant difference in patients lives by providing skilled care in a home setting, with a structured Monday through Friday schedule and the potential for contract extension.
Schedule:
- 13-week contract, full-time hours
- Monday through Friday
- On-call likely as part of the schedule
- Structured daily visit expectations
Key Responsibilities:
- Deliver high-quality in-home nursing care and skilled clinical interventions
- Complete patient assessments, ensuring accurate and timely OASIS documentation
- Collaborate with physicians and interdisciplinary care teams to create and execute individualized care plans
- Provide wound care, manage medications, educate patients, and monitor disease progression
- Keep clear communication with the clinical team regarding updates or changes in patient condition
Qualifications:
- Active RN license
- At least 1 year of recent experience in home health nursing
- OASIS documentation expertise required
- Proficiency with HomeCare HomeBase is strongly preferred
- Proven ability to manage structured daily schedules and maintain accurate records
- Reliable transportation and valid drivers license
