Job Description

An opportunity is available for an experienced and dedicated Registered Nurse (RN) specializing in home health care to support patients in their own homes. This contract position requires strong clinical expertise, including proficiency with OASIS assessments, and a passion for delivering high-quality patient-centered care.

Key responsibilities include:

Providing comprehensive nursing care, including initial and ongoing patient assessments such as vital signs, medication management, and wound care
Developing and implementing individualized care plans tailored to patient needs
Educating patients and families on disease management, treatment options, and preventive measures
Collaborating with physicians, therapists, and other healthcare professionals to ensure coordinated care
Accurately documenting all patient care activities in electronic medical records (EMR) systems
Monitoring and reporting any changes in patient conditions to the supervising physician
Ensuring full compliance with applicable state and federal home health care regulations

Qualifications desired:

Active Registered Nurse (RN) license
Minimum of 3 months of home health experience utilizing OASIS assessments
Experience with Home Care Home Base (HCHB) software preferred
Strong clinical skills, including IV therapy, wound care, and medication administration
Proficient computer use with EMR systems
Excellent communication and interpersonal skills
Ability to work independently and manage time effectively
Valid drivers license and reliable transportation
