Your next nursing opportunity starts here. We are seeking a skilled and dedicated Registered Nurse (RN) with proven Home Health experience for a 13-week full-time contract role. This position is ideal for RNs confident in clinical practice, with precise documentation skills and the ability to thrive within a set schedule. Youll make a tangible difference for patients every day while becoming an essential member of a supportive and professional home health team.
Schedule:
- 13-week contract, with the potential to extend
- Full-time hours, Monday through Friday
- On-call responsibilities may be required
Key Responsibilities:
- Deliver in-home nursing care and skilled clinical interventions
- Accurately complete and submit OASIS documentation in a timely manner
- Collaborate with physicians and interdisciplinary care teams to establish tailored care plans
- Perform wound care, medication administration, patient and caregiver education, and chronic disease monitoring
- Communicate patient updates, status changes, and care needs with the clinical team
Qualifications:
- Active RN license in good standing
- Minimum of 1 year recent experience in home health nursing
- Required: OASIS documentation proficiency
- Familiarity with HomeCare HomeBase (preferred)
- Strong organizational skills with the ability to maintain a structured daily visit schedule
- Must possess reliable transportation and a valid drivers license
