Job Description

An opportunity is available for a Registered Nurse (RN) with expertise in home health nursing to provide exceptional patient care in a contract role. This position requires experience with OASIS and a passion for delivering personalized nursing services in patients homes.

Key Responsibilities:

Deliver comprehensive nursing care including assessments, vital signs monitoring, medication management, and wound care.
Perform initial and ongoing patient evaluations to develop individualized care plans.
Educate patients and families on disease management, treatments, and preventive health measures.
Collaborate with physicians, therapists, and other healthcare providers to coordinate care.
Document patient care activities accurately and timely in electronic medical records.
Monitor patient conditions and report significant changes to supervising physicians.
Ensure compliance with home health care regulations at the state and federal levels.

Qualifications:

Active Registered Nurse license.
Minimum of 3 months home health experience using OASIS documentation.
Experience with Homecare Homebase (HCHB) EMR system is highly desirable.
Strong clinical skills including IV therapy, wound care, and medication administration.
Proficient in electronic medical records.
Excellent communication, interpersonal skills, and ability to work independently.
Valid drivers license and reliable transportation.
